Patient Safety or Care Concerns

All Voyage staff are encouraged and expected to inform Voyage (with 24 hours of occurrence) if an unexpected patient incident occurs related to care, treatment, and services provided, including errors, safety hazards, injuries, and sentinel events, regardless of whether the incident resulted in an adverse patient outcome.

The reporting of this information aides in the reduction of the errors, injury, and safety risks and is for record keeping and educational purposes only. No punitive action will be taken for reporting any events or concerns related to patient safety or patient care.

If you feel your concerns about patient care and safety can not be resolved through your agency or the facility of your assignment you are encouraged to contact the Joint Commission's Office of Quality Management to report any concerns or register a complaint about a Joint Commission certified organization by calling (800) 994-9910 or emailing complaint@jointcommission.org.
