All Voyage staff are encouraged and expected to inform Voyage (with 24 hours of occurrence) if an unexpected patient incident occurs related to care, treatment, and services provided, including errors, safety hazards, injuries, and sentinel events, regardless of whether the incident resulted in an adverse patient outcome.
The reporting of this information aides in the reduction of the errors, injury, and safety risks and is for record keeping and educational purposes only. No punitive action will be taken for reporting any events or concerns related to patient safety or patient care.
